Required Appointment Forms

The forms below should be completed, printed, and returned to Human Resources on or before your first day. Any questions regarding these forms should be directed to Human Resources at 303.335.2067.  Information regarding Benefits can be found under "resources" on the right side of the screen.

Form I-9 Employment Eligibility Verification

Appointments are contingent upon verification of eligibility to work in the United States. Newly hired employees must complete and sign Section 1 of this form no later than the first day of employment. This is a printable version. You will need to print it out, complete it and return it with your appointment paperwork.

Refer to page 9 of this document for a list of acceptable citizenship documents. At least one or more forms must be presented on your first day of employment. Appointment paperwork cannot be processed without this verification.

Form W-4 IRS Withholding

The payroll office uses the address on the W-4 for purposes of issuing W-2's. The home address listed on the W-4 is used in order to compute federal and state taxes.
